Special Nutrition Needs Request Procedure
Our Food Service Department is committed to providing meals that meet the nutritional needs of all students. If a student requires a meal modification due to a medical condition, the following procedure must be followed:
- Initial Request
- Parents/guardians or students may verbally inform the school or food service staff of a special dietary need.
- We will honor the request and make reasonable accommodations while awaiting official documentation.
- Medical Documentation
- A Special Dietary Request Form must be completed and signed by a licensed healthcare provider each school year.
- This form is available on the district website or can be requested from the school office or food service department.
- Submission & Processing
- The completed form should be submitted to the school nurse or food service office as soon as possible.
- Once received, the food service team will review the request and implement the necessary dietary modifications.
- Ongoing Support & Review
- Meal modifications will continue as outlined in the medical documentation.
- If dietary needs change, a new form must be submitted with updated medical information.
For any questions or concerns, please contact the Food Service Department. We are committed to ensuring every student has access to safe and nutritious meals.

Tree Nuts, Peanuts, and Seeds
Food service does not use peanuts or tree nuts in meals, but we do not guarantee all our products come from nut free facilities. We do sometimes use seeds such as sunflower and sesame seeds. Please use the above form to notify us of any seed allergies so that we can accommodate your student.
